Neonatal diabetes occurs in first six months of life, which can be transient or permanent. In contrast to diabetes mellitus in children, regarded as type 1 due to underlying autoimmunity in predetermined genetic background. Neonatal diabetes is a distinct entity with a multitude of underlying mechanisms, due to a single gene defect and more than 20 genes are recognized. It leads to either structural or functional defect in the pancreas. Neonatal diabetes can be associated with extra-pancreatic features which depend on type of underlying gene defect. Diagnosis depends on clinical review with molecular genetic study. The understating of these mechanisms at molecular level is increasing and a number of genetic defects are recognized. Management of diabetes depends on presentation. Insulin is usually the cornerstone of therapy, however some genetic defects respond better to oral hypoglycemic agents and insulin can be switched carefully to oral hypoglycemic agents. In selected cases diagnosed associated extra- pancreatic problems should be appropriately managed with a holistic team approach.
Irfan Ullah is currently working as a Senior Specialist of Pediatric Endocrinology at the University of Sultan Qaboos, Muscat. He has completed his Fellowship from College of Physician and Surgeons, Pakistan, in 2002. Later, he received his Membership from Royal College of Physician of Ireland. He completed his graduation from University of Punjab, Lahore, Pakistan. He then worked at the Riphah University, Pakistan and served as a Senior Specialist. He served as an Associate Professor at the Oman Medical College, Oman, affiliated with West Virginia University, USA. He has authored several publications in various journals. His publications reflect his research interests in neonatal diabetes, childhood osteoporosis and monogenic obesity. He is serving as a Member in Association of Asia Pacific Pediatric Endocrine Society and Arab Society of Pediatric Endocrinology and Diabetes. He is currently involved in scholarly project of thyroid hormone resistance.
