Abstract:

Organizations began reporting medical errors since 1820. The purpose of these organizations is to find, to report and prevent medical errors. The aim of this study is to introduce methods for prevention of medical errors. This study is a review that is done by referring to databases such as Pub med ,Google scholar and Scopus. Articles with the key words of dental error, medical error, misdiagnosis, mistreatment and prevention were studied from 2000 to 2015. Results: Classification of medical and dental errors helps to remove flaws in the healthcare system. Classification of medical and dental errors is based on error type (active, hidden) ,time of their occurrence, time of health care (diagnosis treatment monitoring ), other reasons (defect in the equipment or the health care system, etc.) and error reporting system (sentinel, no harm event and near miss) Conclusion: Taxonomy of medical errors works in areas such as to explain the error, to predict when and where the error occurred, and to provide solutions for error prevention, So the classification of medical and dental errors and timely reporting of errors can help to reduce the incidence of medical errors.
