Gingival recession is defined as the apical displacement of gingival margin in relation to the cementoenamel junction (CEJ). Many treatment modalities have been used to achieve root coverage. These are laterally positioned flap, double papilla flap, coronally advanced flap (CAF), free gingival grafts, subepithelial connective tissue grafts (SCTGs), guided tissue regeneration, acellular dermal matrix allografts and a combination of the above. Among these procedures, the subepithelial connective tissue graft (SCTG) is considered as the gold standard because of its high predictability for root coverage, dual blood supply, superior esthetics and resultant increase in the width of keratinized gingiva. Since then, many variations have been developed in managing the SCTG and they continue to expand. There are many factors which can influence the outcome of SCTG. Although the success rate of SCTG procedures is high, factors for predicting treatment outcome have not been fully identified and needs to be addressed. The aim of this systematic review was to analyze and identify the factors such as the nature of defect/cervical abrasion defect, recession type defect, thickness of flap, different flap designs for the recipient site, different harvesting techniques to procure SCTG from the donor site, presence/absence of epithelial collar, thickness of graft, flap tension, different suturing techniques for SCTG and recipient site flap, effect of smoking on the outcome of SCTG for management of Miller's class I and class II isolated gingival recession defect.
