Efficacy of platelet-rich plasma in reduction of the resorption of the alveolar cleft bone graft-A comparative study

Abstract:

Alveolar bone grafting has become an essential processing the treatment of cleft lip and palate. Autologousiliac bone graft is used preferably because of its sufficient quantity and high osteoinductive potential. However, even with iliac bone, insufficient osteoregeneration may occur due to several factors such as the patient?s age, cleft width, influence of functional stress, and others. Platelet-rich plasma (PRP) extracted from autogenous blood contains many growth factors, such as platelet- derived growth factor (PDGF), vascular endothelial growth factor (VEGF) and transforming growth factor beta (TGF-b). PRP can accelerate bone regeneration and enhance bone formation by accompanying autogenous bone graft or bone substitutes. In oral and maxillofacial surgery, PRP effectively influences wound healing, implant placement, and reconstructive surgery of mandibular defects. PRP is considered to accelerate bone regeneration, and consequently, it provides a mature and thick bone bridge following secondary bone grafting the alveolar cleft. The purpose of the present study was to evaluate the usefulness of PRP for secondary autogenous bone graft in the alveolar cleft.The patients divided into two equal groups. In group 1: the patients treated by bone graft with Platelet-Rich Plasma. In group 2; the patient treated by bone graft only. Postoperative, The osseous resorption was evaluated using the method described by Abyholm et al. the results of the current study showed that the PRP was effective in decease the incidence of bone resorption in the alveolar cleft graft.

Dr. Ayman Hegab has completed his Ph.D. at the age of 32 years from Al-Azhar University and he is a Lecturer of Oral & Maxillofacial Surgery, Faculty of Dental Medicine, Al-Azhar University, and Cairo. Egypt. He is a Member of Egyptian Society of Oral & Maxillofacial Surgeons& a reviewer of Drugs in Dentistry-Evidence Healthcare Canada Journal- Golf Issue. He has published more than 10 papers in reputed journals. He is one of AEEDC Young Researcher Award Participants for 2012. He has three posters presentation and one oral presentation in AEEDC2012& Oral Presentation in The 2 nd Qatar Dental International Conference and the 8 th Gulf Dental Association Conference 2012.
