A pigmented birthmark that initially occurs at birth or within the first year of life is referred to as a congenital nevus, sometimes known as a mole. One percent to two percent of people have these. These moles can appear anywhere on the body, however they are typically found on the trunk or limbs. However, a tiny percentage of congenital nevi may subsequently progress to skin cancer (melanoma), posing a risk to health. The size of the nevus affects the likelihood of developing melanoma. The Large Congenital Melanocytic Nevus (LCMN), which affects around 1 in 20,000 infants globally, is a rare type of congenital mole. In adulthood, LCMN are wider than 20 cm2, or around 8 inches. Giant Congenital Nevi (GCN), which are congenital moles that are larger than 40 cm2 in adulthood, are another name for these that certain medical professionals may use.
LCMN and GCN are substantially narrower at birth, but they develop in accordance with a child's growth to reach a breadth of over 20 cm2. The likelihood of these moles developing melanoma is highest. A plastic surgeon can assist with the surgical excision and any restoration required lessening scarring and enhancing aesthetics when removing these very large moles.
Large/giant CMN is noticeable at birth and typically covers a portion of the trunk. Less frequently, they occur on the head, neck, and limbs. Large CMN affected areas have been labelled as "cape," "bathing trunk," "tippet," or "garment" CMN as a result of their respective distributions. Lesions can have undefined or specific borders. A number of tiny CMNs will typically be present alongside the major CMN in roughly 75% of instances. Although linguistically and molecularly, it is more fair to speak of "disseminated" lesions, the term "satellite" is frequently used to describe isolated small or medium CMNs or tardive nevi in the presence of a big or giant CMN. These extra, smaller, dispersed CMN may already exist at birth or may become significantly more numerous throughout the first few years of life. It is important to know how many of these multiple CMN there are because higher numbers (>20) have been linked to neurological abnormalities.
In general, CMN might be flat, elevated, or even considerably thickened at birth. Its hue ranges from tan to brown to darkbrown to black, and it is rarely blue. Color may be relatively constant throughout or have different hues, such as various tones of brown, black, red, or blue. Hair may or may not be present at birth, and it may or may not grow as the kid gets older. The texture may be smooth, heavily nodular, or cobblestone-like. The texture of this hair might be fine (vellus) or, more frequently, course (terminal). In big scalp CMN, there may be a "cerebriform" or brain-like texture. Regions of the nevus may have overgrowths of fatty or nerve tissue, referred to as lipomatous or neurotised areas, respectively. These are the characteristics that are most frequently observed in larger CMN.
CMN may evolve over time to become darker or lighter, more or less colour heterogeneous, and with a different surface texture. Additionally, superimposed nodules which are typically benign but need monitoring can form in CMN. The CMN-affected skin may be dry and develop underlying eczema, causing sporadic or on-going itching (pruritis). Additionally, CMN may have fewer sweat glands than unaffected skin, which could lead to overheating episodes or more sweating in other parts of the body as a coping mechanism. Particularly in the vicinity of the sides, limbs, and buttocks, regions of greater CMN may have noticeably less fat under the skin.
Due to the skin's insufficient development during this time, transitory erosions or ulcerations may appear over large CMN at delivery or within the first few weeks of life. Healing typically takes days to weeks. Rapidly expanding "proliferative nodules" in the CMN that mimic melanoma but have benign properties upon examination are also noticeable throughout the infancy period. It is advised to have a dermatopathologist with experience in pigmented lesions evaluate these nodules to prevent needless surgery and perhaps harmful adjuvant therapy if they are mistaken for melanoma. In the first few years of life, big CMN can also exhibit significant colour lightening, particularly when affecting the scalp.
Treatment is not necessary for the majority of congenital melanocytic nevi. Each month, check the mole or moles. To see if there have been any changes, it may be helpful to take a photo of the mole(s) with your smartphone or digital camera.
In childhood consult a doctor if there are any changes, such as areas of bleeding or crusting, new rough patches, spots that change colour, new pain or itch, changes in shape, or a sudden change in size. Doctor might advise getting a skin biopsy or taking a sample. Complete excision of some congenital nevi may be advised. In younger children or in any child who is fearful or anxious about the procedure, general anaesthesia can be necessary.
The family may decide to completely remove the lesion given the minimal chance of melanoma developing in conjunction with a congenital melanocytic nevus. This may also be advised if the nevus is Located in a location that may be challenging to observe, such as the scalp or buttocks; In a youngster who exhibits excessive anxiety or worry around the development of the congenital melanocytic nevus. A mole must be removed surgically. The mole removal will leave a scar, but it will significantly lower the likelihood of developing melanoma. This should be taken into account while choosing whether or not to remove it. There are no safe ways to eliminate melanocytic nevi with lasers or other procedures.
The arsenal of plastic surgery procedures is still the mainstay of treatments. The gold standard is to replace the skin in its entirety, even if partial-thickness grafts or ablations utilising dermabrasion or curettage have been performed in the past and are still occasionally relevant. By implanting expanders either next to or inside a donor graft site, or by naturally forcing expansion from nearby locations, replacement skin can be produced from other zones. The majority of nevi cannot currently be replaced with synthetic dermis. For broad, technologically inaccessible, and homogeneous nevi, simple surveillance is a possibility. Regardless of the physical therapy methods, psychological support for patients and families is strongly advised. Due to the very individual distributions and combinations of CMN's textures, nodules, and other characteristics, every treatment plan is different.
